OHL Case Manager Honored By UNITY
OHL Community Supportive Services (CSS) Case Manager Tiba Hankton was awarded Outstanding Case Manager of the Year award at UNITY Annual Meeting! This award recognizes the outstanding work Tiba has done in the past year in assisting clients maintain stable housing.
Tiba has been with OHL for 12 years, working as a case manager for various program. She is now in OHL’s Community Supportive Services department, which focuses on moving individuals and families to stable, secure housing, and keeping these families and individuals housed and engaged in case management. Case management is a process in which the OHL case manager and client work collaboratively to assess, plan, implement, coordinate, and monitor the services required to meet the client's health and human service needs. All program participants are required to contribute a portion of their rent. This is based on the individual client’s income level. OHL operates housing programs for the chronically homeless, as well as homeless individuals who have special needs such as mental, physical, or developmental disabilities, substance abuse issues, or HIV/AIDS. It is our goal to successfully house individuals as quickly as possible and begin engaging them and their families in services.
